1. Eligibility

You must be at least 18 years of age and a resident of a U.S. state where our services are available to use Eros MD. By using our services, you represent and warrant that you meet these eligibility requirements. Our telehealth services are only available to individuals located in states where our licensed providers are authorized to practice.

2. Nature of Services

Eros MD provides telehealth-based consultations and personalized health protocols related to men's vitality, hormone optimization, and general wellness. Our services are provided by or under the supervision of licensed healthcare providers.

Our services do not replace your primary care physician or specialist. You should not discontinue any current treatment without consulting your existing healthcare provider. In the event of a medical emergency, call 911 or go to your nearest emergency room immediately.

3. Telehealth Consent

By using our services, you acknowledge and consent to receiving healthcare services via telehealth technology. You understand that:

• Telehealth involves the delivery of health services using electronic communications and information technology.
• There are potential risks including, but not limited to, technical difficulties, interruptions, or limitations of telehealth technology.
• Your provider may determine that telehealth is not appropriate for your condition and may recommend in-person care.
• Information transmitted may not be sufficient to allow for appropriate medical decision-making in all cases.

4. User Accounts and Information

When you submit an intake form or create an account, you agree to provide accurate, current, and complete information. You are responsible for maintaining the confidentiality of any account credentials and for all activity that occurs under your account. You agree to notify us immediately of any unauthorized access or use of your account.

The content on our Site is provided for informational purposes only and does not constitute medical advice, diagnosis, or treatment. Always seek the advice of a qualified healthcare provider with any questions you may have regarding a medical condition.

6. Prescriptions and Medications

If prescribed medications as part of your protocol, you acknowledge that:

• All prescriptions are issued at the sole discretion of your assigned licensed provider.
• You will use prescribed medications only as directed.
• You will disclose all current medications, supplements, and medical conditions to your provider.
• You will report any adverse reactions or side effects to your provider immediately.

7. Payment and Billing

By purchasing services through Eros MD, you agree to pay all fees and charges associated with your selected protocol or consultation. All fees are stated in U.S. dollars. We reserve the right to change our pricing at any time, though changes will not affect active subscriptions until their next renewal period. Refund policies are outlined at the time of purchase.

8. Cancellation

You may cancel your services at any time by contacting us at info@erosmd.com. Cancellations are effective at the end of your current billing period. We do not provide prorated refunds for partial billing periods unless required by applicable law.

9. Intellectual Property

All content on the Site, including text, graphics, logos, images, and software, is the property of Eros MD or its licensors and is protected by U.S. and international copyright, trademark, and other intellectual property laws. You may not reproduce, distribute, modify, or create derivative works from any content without our prior written consent.

10. Prohibited Conduct

You agree not to:

• Provide false or misleading information in any intake form, consultation, or communication.
• Attempt to access another user's account or personal health information.

To the maximum extent permitted by law, Eros MD and its officers, directors, employees, and agents shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or related to your use of our services. Our total liability shall not exceed the amount you paid for services in the twelve (12) months preceding the claim.

13. Governing Law

These Terms are governed by and construed in accordance with the laws of the State of Wyoming, without regard to its conflict of law provisions. Any disputes arising under these Terms shall be resolved in the state or federal courts located in Sheridan County, Wyoming.
